چکیده
First Admission The patient's blood pressure was found to be 150/ 110 mm Hg, and pelvic exam revealed a large pelvic mass. Intravenous pyelogram showed normal kidneys and collecting system; a pelvic soft tissue mass indented the bladder. Serum potassium was 2.8 mEq/liter, random peripheral plasma renin activity (PRA) was 52 ng angiotensin I (ANG I)/ml/hr (normal upper limit on a low salt diet = 9.1 ng ANG I/ml/hr), and urine vanillylmandelic acid (VMA) was 8.2 mg/24 hr (upper limit of normal = 8 mg/24 hr). An exploratory laparotomy revealed a tumor in the fallopian tube with perito-

Renovascular hypertension has its experimental counterpart in the two-kidney, one clip model (Goldblatt hypertension). From the study of this model, a general pathophysiological scheme has evolved suggesting that temporal stages in the development and maintenance of hypertension are regulated by complicated hormonal and neural interrelations. The central roles played by the renin-angiotensin sy...

Hypertension, defined in different ways by various investigators over a period of many years, has been shown to be a major risk factor for stroke. In fact, the strength of the evidence suggests that hypertension causes stroke. But by what mechanisms? The value of treating chronic hypertension to prevent stroke is well established, but what should be done about blood pressure elevations in the se...
متن کاملClinical Conference Pressor Systems in Hypertension and Congestive Heart Failure Role of Vasopressin Principal Discussant
Elevated peripheral vascular resistance, which characterizes hypertension and congestive heart failure (the latter regardless of absolute blood pressure level) is maintained to a large extent by the combined effects of three major neurohormonal pressor mechanisms: the renin-angiotensin system, the sympathoadrenal system, and arginine vasopressin. Blockade of one of these mechanisms may lead to ...
